This application form is for nurses and midwives applying for a scholarship in 2025.

- Official Enrolment Confirmation (this must include individual units of study, and their weighting); and
- Financial Statement or Tax Invoice (this must include the fee for each individual unit within your course); and
- Validation Statement (template available from the Tasmanian Nursing and Midwifery Scholarship website).
For Professional Development or Vocational Education and Training:
- Official Enrolment Confirmation; and
- Financial Statement or Tax Invoice; and
- Validation Statement (template available from the Tasmanian Nursing and Midwifery Scholarship website)
The Official Enrolment Confirmation and Financial Statement or Tax Invoice MUST contain your name, and the name of the University or Education Provider.
