    Use this form to report concerns of inappropriate behaviour by a staff member, volunteer, visitor, patient or other person within Tasmania’s health care services or premises.

    It can be used by patients, families, staff or members of the public who may have experienced or observed behaviour that caused concern or may be inappropriate.

    The types of behaviour that you might use this form to report include:

    • Aggressive or violent behaviour
    • Breach of healthcare rights
    • Behaviours or interactions that cross professional boundaries (e.g., age-inappropriate or sexualised language, physical contact outside of clinical need, preferential relationships)
    • Any sexualised behaviours or interactions with, or in the presence of a child
    • Bullying and/or intimidation
    • Sexual harassment
    • Any other behaviour that you thought was inappropriate or caused you concern.

    Our staff are only provided with, or have access to, the information that is necessary for them to carry out their functions within the Department of Health. All staff are bound by confidentiality requirements.

    Please note, there may be a need or requirement to disclose some or all of this information. In certain circumstances such as serious misconduct or potentially criminal behaviour, the Department has an obligation to disclose the behaviour to one or more of the following organisations:

    • Tasmania Police
    • Strong Families Safe Kids
    • The Registrar to Work with Vulnerable People
    • Australian Health Practitioner Regulation Authority (AHPRA)
